Emergency Area – Kaster Medical Technology

In the Emergency Area, Kaster Medical Technology supports hospitals, trauma centers, and air ambulance services in the management of patients in hemorrhagic shock and in situations with a high risk of mortality.
Specifically, Kaster specializes in supporting the dissemination and proper implementation of the REBOA (Resuscitative Endovascular Balloon Occlusion of the Aorta) technique and the COBRA-OS® device, a temporary endovascular occlusion system for the aorta and great vessels.

The Challenge: Noncompressible Hemorrhages and Hemorrhagic Shock

Noncompressible hemorrhages, especially in the thoracic, abdominal, and pelvic regions, are a leading cause of preventable death in trauma patients. In these settings, traditional hemorrhage control strategies (external compression, immediate surgery, interventional radiology) may be insufficient or unavailable in a timely manner.

Hemorrhagic shock rapidly leads to hemodynamic instability, hypoperfusion of vital organs, and irreversible worsening of clinical outcomes if not reversed with effective and timely treatment.

The use of REBOA requires:
  • Dedicated operator training.
  • A defined institutional pathway (protocols, internal guidelines, multidisciplinary team).
  • The availability of specific devices, designed to reduce invasiveness and the risk of vascular complications.

REBOA Technique: Resuscitative Endovascular Occlusion of the Aorta

The REBOA technique involves the percutaneous introduction of a balloon catheter into the aorta, which is inflated to temporarily occlude the vessel. This results in:

  • Temporary control of hemorrhage downstream of the balloon.
  • Increased flow and pressure to the heart and brain (proximal perfusion).
  • A resuscitative "bridge" that allows time for definitive treatment (surgical or endovascular).

REBOA is a lifesaving technique that has revolutionized the management of non-compressible hemorrhages, particularly in polytrauma patients in shock,in selected cases of severe postpartum hemorrhage, and, in advanced settings, also in the management of refractory non-traumatic cardiac arrest, as an option to improve perfusion during CPR and increase ROSC rates.

COBRA-OS®, the new generation

Kaster Medical Technology is the exclusive distributor in Italy of COBRA-OS®, a balloon occlusion system for large vessels, manufactured by Front Line Medical Technologies Inc.
COBRA-OS
COBRA-OS® integrates a series of features designed for use in critical settings and to minimize the risk of complications related to arterial access:
  • 4-French profile: allows for more conservative access than larger-caliber systems, helping reduce the risk of limb ischemia, hematomas, and pseudoaneurysms.
  • Flexible and atraumatic distal J-tip with rigid internal guidewire to facilitate secure placement in the aorta.
  • Compliant balloon with proximal and distal necks, designed to fit the vessel lumen.
  • Markers for Zone 1 (48 cm) and Zone 3 (28 cm) for faster and more precise placement based on the clinical indication.
  • Radiopaque (RO) markers for clear visualization under fluoroscopic guidance.

COBRA-OS® is available as a sterile, single-use kit,designed for immediate use in emergency situations, containing:

  • COBRA-OS® Device.
  • Dedicated 10 ml syringe.
  • Custom Sheath Introducer Kit 4 Fr, including a 0.018" floppy-tipped guidewire, a kink-resistant 4 Fr introducer, and a 21G 7 cm blunt angiographic needle.
The device is supplied sterile, single-use, with a dedicated introduction kit and syringe included.

Technical training with a dedicated simulator

Kaster Medical Technology supports centers wishing to implement or consolidate the REBOA technique and the use of COBRA-OS® within their programs by providing a dedicated simulator for technical training.
